Biggest consistent issue I've come across in the U.S. is people lack lane discipline but that's true here as well.
Drivers in Massachusetts are truly terrible in my experience. We went to Cape Cod a few years ago and at one point we were in very heavy traffic and ambulance was trying to get through and people simply wouldn't get out of the way.
There's your problem right there. Habits vary by state, even locality.
Lane discipline is sorely lacking as a whole though.
